You live within the TW FHT catchment area (please see map below)
You are insured for care in Ontario (OHIP or interim federal health)
You register with Health Care Connect and do not have a primary care provider (physician or nurse practitioner)

Due to high demand, our current response time is approximately 4 to 8 weeks. Our wait times for an intake appointment may vary depending on the current capacity and availability of our providers. An email confirming the status of your submission will be sent to you once your application has been processed. Kindly refrain from contacting the clinic directly to inquire about the status of your application, as Reception staff are unable to provide updates. Please note, applications are processed in the order they are received.

Why do I need to sign a Patient Enrollment Form (Roster Form)?
Family Health Teams in Ontario provide care to enrolled patients. By signing this roster form, you agree to receive care from your assigned PCP and the team at TW FHT. For more details, review the New Patient Information.
Yes, you can apply. Fill out the online application and include your current provider’s name and retirement date in the comments. Also, please complete the Health Care Connect application to be considered.
Not always. At your first appointment, you and your new primary care provider will review your medical history and decide whether your previous records are needed.
Many of your past lab tests and results can already be accessed through a secure provincial system using your health card. If you have specific reports, forms, notes, or test results that you think are important, please let your new PCP know during your visit. If you and your PCP decide that your old medical records would be helpful, you will need to request them from your previous healthcare provider. It is the patient's responsibility to request medical records. Please note that your previous provider may charge a fee for copying and releasing the medical records.
Family members living in the same household as a current patient may be accepted. Complete the online form and include the current patient’s name and PCP in the comments. Your PCP will decide whether they will accept the family member into their practice.
